Optiform endorses the Australian Charter of Healthcare Rights and we support the rights of Participants to whom we provide supports to:
- access health care;
- receive safe, high quality health care;
- make choices about, comment on their health care needs and have their concerns considered and addressed where possible;
- be shown respect and be treated with dignity and consideration;
- be included in decisions that impact them;
- be informed about services, treatment, options and costs in a clear and open way; and
- privacy and confidentiality with respect to personal information.
Participants’ Rights
Optiform is committed to ethical practice and we support the rights of the Participants to whom we provide Supports to:
- participate in community life;
- fair and respectful treatment;
- make choices, comments, offer suggestions, raise issues or complaints about Optiform’s services, policies or operations;
- be shown respect with regard to property or assets;
- request changes to the ways Optiform offers Supports, or change to a different service provider; and
- receive clear information about Optiform’s Policies, Services and Supports.
Participants’ Responsibilities
Optiform requests Participants receiving our services to:
- refrain from any violent or aggressive conduct;
- treat Optiform employees appropriately and with respect at all times;
- encourage their supporters to treat Optiform employees with respect and courtesy at all times;
- provide open communication that allows Supports to be delivered and developed;
- provide Optiform with the information that is necessary for us to safely and efficiently deliver Supports;
- inform Optiform as soon as possible of any problems, concerns or complaints with the delivery or development of Supports;
- inform Optiform if:
- your NDIS plan is suspended or replaced with a new NDIS plan;
- your plan payment method changes; or
- you stop being a participant in the NDIS;
- abide by Optiform’s policies on smoking, including no smoking near our employees or premises;
- take reasonable care not to damage Optiform’s property or assets; and
- notify Optiform if Supports are no longer required, either on a one-off or ongoing basis.
