Optiform is a registered provider for the following funding options. We are happy to help you through the funding process.

MEDICARE

Mental Health Care Plans (MHCP)

Chronic Disease Management Plan (formerly Enhanced Primary Care Plan)

These plans are designed for clients who require ongoing care from a multidisciplinary team or have a chronic medication condition which has been/is likely to be present for six months or longer. Clients that have been given a Chronic Disease Management Plan from their GP will be eligible to receive a Medicare rebate for a combined total of 5 allied health appointments (including Speech Therapy and Occupational Therapy) per calendar year. Clients that have been given a Mental Health Care Plan from their GP will be eligible to receive a Medicare rebate for a minimum of  6 Occupational Therapy appointments per calendar year.  Rebates will be processed following full payment of your account on the day of your appointment.

Helping Children with Autism (HCWA)

This funding scheme is for early intervention for children who have a diagnosis of Autism Spectrum Disorder or other Pervasive Development Disorders. Clients are provided with up to $12,000 to use across two financial years on registered speech pathology, psychology and occupational therapy. In order to access the HCWA, your child needs to be between the ages of 0-7 years, and you need a diagnosis from approved psychologists, speech pathologists of paediatricians. We are eligible to provide therapy services under this funding.

NDIS

The National Disability Insurance Scheme (NDIS) is the new way of providing support for Australians with disability, their families and carers. The NDIS supports people with disability to build skills and capability so they can participate in the community and employment.

To access the NDIS, a person must meet Australian residency requirements, be aged under 65 at the time they apply for the scheme, and demonstrate they have a permanent disability that affects their everyday life.

Optiform is currently registered to provide therapeutic supports through Occupational Therapy, Speech Pathology and Behavioural Therapy.

Our NDIS registration covers the following Core Support areas:

  • Early Childhood Supports
  • Assistive Equip-Recreation
  • Improved Daily Living – Therapeutic Supports
  • Improved Relationships – Behavioural Supports

WorkCover

If you’re injured at work and need medical treatment or time off work, you may be eligible for compensation which may include time off work and treatment expenses.

Optiform is registered to provide therapeutic supports for WorkCover clients. Optiform bill the insurance company directly, so there is no charge for you, the client. Please bring your doctors referral and WorkCover claim number to your first appointment. If you are suffering from a work related issue, you should speak to someone at your work place and lodge a WorkCover claim.

If you are being treated under WorkCover your therapist will liaise with your case manager to formulate a plan for the best possible treatment for your condition. For more information, click here.

DVA

The Department of Veterans’ Affairs (DVA) offers a range of grant programs to assist with providing support to veterans and their families.  These grants support the commemoration of Australia’s wartime heritage and the development and extension of innovative and sustainable new services to build community capability.

TAC

The TAC is a Victorian Government-owned organisation set up to pay for treatment and benefits for people injured in transport accidents, promote road safety and improve Victoria’s trauma system.  Immediately after a transport accident, TAC will pay the expenses for therapeutic supports such as Speech Pathology and Occupational Therapy. All that is required from you is a TAC claim number, and approval from your General Practitioner.

Private Health Rebates

Private Health Insurance rebates are also available for Speech Pathology and Occupational Therapy appointments.  Optiform is a registered provider under all known Private Health companies. The ability to access this funding is determined by your level of cover and/or if you have selected Speech Therapy and Occupational Therapy as your claimable Allied Health Services.  Please contact your Provider for further information regarding rebate amounts/restrictions.

Optiform uses HICAPS (Health Industry Claims and Payment Services) which automatically processes your claim on the day of your appointment. If you are claiming from your health fund, its contribution will be processed on the spot, and you pay the remaining gap if applicable.
