Optiform will maintain records of all hazards, near misses, incidents and injuries for the purposes of minimising risk of injury and ill-health or the recurrence of an incident.
All Optiform employees are expected to report hazards, near misses and incidents, and these will be addressed accordance with our Policies and in consultation with relevant employees.
If a Reportable Incident occurs, Optiform will notify the NDIS within 24 hours and adhere to the ongoing reporting obligations pursuant to the National Disability Insurance Scheme (Incident Management and Reportable Incidents) Rules 2018.
