Optiform is registered to provide services for people via Medicare, NDIS, TAC, DVA, WorkSafe, and Private Health Insurance.

MEDICARE

As a general rule, Medicare refunds do not cover services such as Occupational Therapy and Speech Pathology. However, the Medicare Allied Health Initiative allows certain clients to access Medicare rebates for allied health services.

Your GP may refer you for allied health services under one of the following plans;

  • Mental Health Care Plans (MHCP)
  • Chronic Disease Management Plan (also known as Enhanced Primary Care Plan /EPC)

These plans are designed for clients who:

  • Require ongoing care from a multidisciplinary team
  • Have a chronic medication condition which has been/is likely to be present for six months or longer.

Clients with one of these plans may be eligible for a Medicare rebate of $53.80 for allied health services such as Speech Pathology and Occupational Therapy. This rebate will be processed following full payment of your account. A plan may cover a combined total of 5 allied health visits per calendar year. The plan lasts for 12 months.

NDIS

The National Disability Insurance Scheme (NDIS) is the new way of providing support for Australians with disability, their families and carers. The NDIS supports people with disability to build skills and capability so they can participate in the community and employment.

To access the NDIS, a person must meet Australian residency requirements, be aged under 65 at the time they apply for the scheme, and demonstrate they have a permanent disability that affects their everyday life.

Optiform is currently registered to provide therapeutic supports through Occupational Therapy, Speech Pathology and Behavioural Therapy.

Our NDIS registration covers the following Core Support areas:

  • Early Childhood Supports
  • Assistive Equip-Recreation
  • Improved Daily Living – Therapeutic Supports
  • Improved Relationships – Behavioural Supports

Private Health Insurance

If your family has Extras cover that includes speech pathology or occupational therapy services, you may be entitled to a rebate from your health fund.

For your convenience, we have a HICAPS terminal installed at the clinic.  This means that if you have appropriate cover, you can process your private health fund claims on the spot and may only need to pay a gap.
